The Item Upgrader in World of Warcraft
The War Within introduced Valorstones, and Harbinger Crests as two currencies to upgrade gear. These currencies can be exchanged at the item upgrader NPCs, Cuzolth and Vaskarn.
Each item can be upgraded to its maximum level for a given slot once. Upgrades to that slot will be cheaper afterward. The pity system, introduced with The War Within, also reduces the extreme variance in upgrades.
Upgrades

There are two major drawbacks with the upgrade system. It can be difficult to determine what items are eligible for upgrade since upgrade vendors don't always show all of them. The upgrade textures are created over the model which can hide enchantment details and other details.
Another disadvantage is that upgrades are costly. Every step in a gear's degree of upgrade will cost a certain amount of Valorstones, or crests, based on the slot and level.
